Earners are prepared as a professional home health case manager or rehab supervisor. Learners will understand how to perform and document OASIS visits, and create and update an evolving patient plan of care to meet patients’ short-term and long-term goals through interdisciplinary care coordination. RNs, PTs, OTs, and STs, are prepared to case manage with appropriate frequencies, treatment modalities, and human resources to achieve positive patient outcomes with efficient cost-saving strategies.
