As a member of the diabetes care team, they have the clinical management skills needed to identify the connection between diabetes and cardiovascular disease, as well as informed and coordinated care of chronic conditions.

Complete 12 modules/webinars on various topics related to cardiovascular disease and diabetes.

Complete a total of 6 hours of self-paced patient case studies that correspond with each module to practically apply the knowledge learned by analyzing each situation and describing the response.

Pass a 50-question post-learning final assessment with a minimum score of 80%.
Standards
ADCES is accredited as a provider of continuing professional development by the American Nurses Credentialing Center's Commission on Accreditation.This program provides 25 contact hours of continuing education credit. Provider is approved by the California Board of Registered Nursing, Provider # 10977, for 25 Contact Hours.
ADCES (AM001) is a Continuing Professional Education Accredited Provider with the Commission on Dietetic Registration. CDR Credentialed Practitioners will receive 25 CPEUs for completion of this activity. CPE Provider Accreditation does not constitute endorsement by CDR of a provider, program, or materials.
ADCES is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education. This program provides 25 contact hours (2.5 CEUs) of continuing education credit.
