Earners have received an introduction to the values, theories, frameworks, principles and skills of person-centred planning and case management – tools. Earners are effective at determining individual need and plan with empathy and ethics. Earners can implement person-centred care frameworks across a range of disciplines, using effective stakeholder engagement, coordination and communication skills as well as tools to manage caseload, to deliver the best community health outcomes for clients.
