This credential recognizes successful completion of the 18.5 hour Anticoagulation Therapy Management for Pharmacists Certificate Program. Program topics include monitoring and managing anticoagulation therapy patients and clinics; hemostasis; comparing the indications, mechanism of action, appropriate dosing, and appropriate monitoring for anticoagulant medications for various indications; and development of a complete anticoagulant treatment plan for patients with various indications.

Earning Criteria

In order to earn credit for this course, participants must complete all course case studies and quizzes with a score of 70% or higher. Two chances to pass each quiz/assessment will be given. If a passing score is not earned by the second attempt, the participant must re-enroll in the course. Upon successful completion of all coursework, participants must complete a course evaluation form. Upon successful completion of the course, participants will be awarded a certificate of completion.
Standards
This program is approved for 20.0 general hours of continuing education for Pharmacist licensure renewal and Consultant Pharmacist licensure renewal by the Florida Board of Pharmacy.
