Earning Criteria

One of the NEPQR Primary Care Modules that teaches learners how to: 1. Recognize appropriate use of common dementia assessment tools. 2. Identify evidence-based dementia interventions and protocols for the chronic illness management of ADRD patients. 3. Recognize the trajectory of ADRD and common support needs of caregivers through that trajectory. 4. Apply patient-centered care concepts of education, communication and comfort to ADRD patients in primary care settings.
