Earners have foundational knowledge and skills necessary for delivering community-based care coordination and chronic disease self-management services to underserved populations. They have increased awareness of health equity and social determinants of health and their role in managing population health. Earners have skills in performing strength-based assessments, providing motivational interviewing, developing co-produced care plans, as well as advocating for changes at local levels & beyond.
